The Shoulder Scan You Probably Weren't Offered

Diagnostic ultrasound can often answer the rotator cuff question in one visit — and unlike MRI, it shows the shoulder moving.

I've spent the better part of two decades evaluating imaging and pain technology — testing it, writing about it for Practical Pain Management, and reviewing it for the journals. This one is personal, because I think a lot of people wait weeks for an answer they could have had in a single visit.

If your shoulder has been hurting for more than a few weeks — if it wakes you at night, if reaching into the back seat makes you wince, if lifting a coffee pot has started to feel like a decision — there's a reasonable chance someone has told you that you may have a rotator cuff tear.

What happens next is usually predictable. You get referred for an MRI. You wait. You find out what it costs. And somewhere in that gap between the appointment and the answer, you spend a few weeks not really knowing what's wrong with your own shoulder.

There is another way to get that answer, and it's one I've spent a good part of my career arguing for: diagnostic ultrasound.

What happens next is usually predictable. You get referred for an MRI. You wait. You find out what it costs. And somewhere in that gap between the appointment and the answer, you spend a few weeks not really knowing what's wrong with your own shoulder.

There is another way to get that answer, and it's one I've spent a good part of my career arguing for: diagnostic ultrasound.

Why ultrasound gets treated as second-best

For years, musculoskeletal ultrasound has been positioned as the budget option — the thing you do when MRI isn't available, or when the patient can't afford it, or won't fit in the machine.

I've argued in the peer-reviewed literature that this framing is wrong, and that it costs patients real time and real money. For rotator cuff tears specifically, ultrasound isn't a lesser substitute for MRI. In the hands of a clinician trained to perform and read it, it's a legitimate first-line imaging tool.

That's not a cost argument. It's a clinical one.

What ultrasound can do that MRI can't

The advantage that matters most to patients is this: ultrasound is dynamic.

An MRI produces an exquisitely detailed picture of a shoulder holding perfectly still. But shoulders don't hurt when they hold still. They hurt when they move — when the arm goes overhead, when the tendon glides under the bony arch above it.

With ultrasound, I can watch that happen in real time. I can ask you to lift your arm and see the tendon move. I can watch for the moment of impingement rather than infer it. I can press directly on the spot that hurts and see what's underneath my fingers while you tell me that's it, that's the spot.

That last part is worth sitting with: ultrasound is the only common shoulder imaging where you and the clinician are looking at the screen together, in the room, while you point at the pain.

Three other things follow from the technology itself:

- You get the answer in the same visit. The scan happens in the office. There's no waiting for a facility appointment, no waiting for a report. - You can be scanned even if you can't have an MRI. Pacemakers, certain implants, severe claustrophobia — none of them rule out ultrasound. - We can compare it to your other shoulder in the same sitting, which is often the fastest way to tell a genuine tear from the ordinary changes that come with having used a shoulder for sixty years.

What ultrasound doesn't do

I'd rather you hear the limits from me than discover them later.

Ultrasound images soft tissue near the surface very well. It does not see inside the joint the way MRI does. If the question is about the labrum, deep joint surfaces, or bone, MRI remains the better tool — and if we scan you and the picture doesn't match your symptoms, the honest next step is an MRI, not a shrug.

Ultrasound is also operator-dependent in a way MRI simply isn't. An MRI scanner produces much the same images regardless of who pushes the button. An ultrasound is only as good as the person holding the probe and reading what's on the screen. That's a real limitation, and it's the main reason the technique has taken so long to be accepted as first-line.

It's also why "do you have an ultrasound machine" is the wrong question to ask a clinic. The better question is "who is doing the scan, and how much shoulder imaging have they actually done?"

The published evidence bears this out. Across the studies I reviewed, diagnostic ultrasound for rotator cuff tears showed sensitivity ranging from 84% to 96%, and specificity from 76% to 93%. At the top of those ranges it is an excellent test — genuinely comparable to what you'd expect from MRI. At the bottom it is merely decent.

That spread is the whole argument in one line. Ultrasound doesn't perform inconsistently because the physics are unreliable; it performs inconsistently because skill varies. The technology has a high ceiling and a mediocre floor, and which one you get depends almost entirely on who is holding the probe.

What this means for your shoulder

If you're weeks into shoulder pain and waiting on imaging, it's worth asking whether a diagnostic ultrasound could answer the question sooner.

Sometimes the answer will be no — the clinical picture points somewhere ultrasound can't see, and MRI is the right call. But often you can walk out of a single visit knowing whether the tendon is torn, whether it's a partial or full-thickness tear, and what that means for whether you're looking at rehabilitation or a surgical conversation.

Knowing changes what you do next. And the weeks you spend not knowing are rarely neutral — shoulders stiffen while they wait.

The larger point

I've spent a long time evaluating pain technology — devices, imaging, neuromodulation — and the pattern repeats: a tool gets categorised early as the cheap alternative, and that label sticks long after the clinical evidence has moved.

Diagnostic ultrasound for the rotator cuff is one of those tools. Used properly, by someone trained to use it, it isn't a compromise. Quite often it's simply the faster way to find out what's wrong with your shoulder.
